As AYLUS grows in members, events, and connections, there grows a new requirement for efficient means of surveys for large groups of people. One such example is the inclusion of a system of annual, electronically submitted waiver forms, which negates the need for the previously inefficient and inconvenient system of organizing waivers at each individual event.
This will have been the second year that we have applied this system; our last form was created and opened on Oct 4, 2022, and this one 1/1/2023. Derek Zhu created a google form to collect the information. The waiver form can be downloaded as a PDF file and printed out, which can then be uploaded along with additional information to a google forms at https://forms.gle/PrYy61DhCwbaMwpCA. Derek Zhu checked, managed and compiled the files to ensure volunteers have their annual waiver filled out before the event. Once the surveyed data is collected, he verified for AYLUS members, and updated periodically on the AYLUS website. One hundred and 147 responses in total have been managed. It takes about 4 hours each time.
