- Name is the student’s name. Be sure to keep the first name and last name correct and consistent for each student. The system will NOT be able to recognize changed spellings.
- Date is when the activity took place (not when you fill this form).
- Requester Email is the student’s email address.
- The “your name” referred in the Event Description is the name of the adviser who is filling this form on behalf of a student (provided the event has been verified by the adviser).
